Короткий огляд (реферат): | Liver resection is widely performed for the treatment of hepatic neoplasms, both benign and malignant as well as for living donation (1). Irrespective of the approach, the procedure can be associated with significant postoperative pain (2). Effective pain control can facilitate early mobilization and reduce postoperative complications (3). One of the reasons for the ERAS society’s recommendation against epidural analgesia in liver surgery is the presence of postoperative coagulopathy (4). Alternative pain control technics required for this type of the patients. |
Опис: | We conducted a single center pilot study to which 39 patients with resection of 40–70% of liver parenchyma were included. All patients received inhalation anesthesia with sevoflurane and intraoperative analgesia with phentanile 0.005% during the surgery. The first group (20 patients) received continuous epidural infusion of lidocaine 1.5%. Second group (9 patients) – received intravenous lidocaine 1.5% 2–4 mg per kg intraoperatively. Third—control group included 10 patients. For analysis of the analgesia we used intraoperative fentanyl consumption, VAS pain score during the first (POD0) and second (POD1) days after the surgery. We also analyzed the time (in min) when first dose of analgetics was administer to the patient after the surgery. |
