Abstract: Aim. The development and implementation into clinical
practice methods of local treatment of purulent-necrotic wounds
in patients with diabetes, based on the use of modern sorbent drugs.
Materials and methods. In 2021, 121 patients with various stages
of diabetic foot syndrome were treated. The main group included
27 patients in whom Ornidasil sorbent was used for local treatment.
The comparison group included 31 patients in whom hypertonic solution
and ointment bandages were used for local treatment.
Research results. The treatment of purulent-necrotic wounds in patients
with type II diabetes with diabetic foot syndrome was analyzed.
Conclusions. Diabetic foot syndrome is a serious complication of
diabetes. Application sorbent consisting of ornidazole and nanosilica
is an effective tool in the treatment of wounds with diabetic foot syndrome.

Abstract: Medical work in wartime has its own peculiarities.
Among the patients admitted to the clinic for treatment, the num
ber of operations for primary treatment of wounds, necroctomy and
amputation of limbs and fingers, removal of foreign bodies (bullets,
fragments, glass, etc.), treatment of combined injuries of soft tissues
with bone fractures, and others is increasing sharply. The use of VAC
and pulse-VAC devices, ultrasonic cavitators, dermatomes, micro-
instruments and suture material for microsurgical interventions is
very important for the preparation of surgical wounds for reconstruc
tive and restorative operations.
Description: Лікувальна робота у військовий час має свої особли
вості. Серед прийнятих на лікування у клініку пацієнтів різко
збільшується кількість операцій з приводу первинної обробки
ран, некректомії та ампутації кінцівок і пальців, видалення
сторонніх тіл (куль, уламків, скла та інших), лікування ком
бінованих травм м’яких тканин із переломами кісток та інші.
Дуже важливе значення для підготовки операційних ран для
реконструктивних та відновлювальних операцій має викорис
тання ВАК та пульсо-ВАК апаратів, ультразвукових кавітато
рів, дерматомів, мікроінструментів та шовного матеріалу для

TRAINING OF INTERNS IN SURGERY: TODAY'S FEATURES
National University of Health of Ukraine named after P.L. Shupika.
The aim: To find the most effective methods of training interns in surgery under conditions of martial law restrictions at the Department of General and Emergency Surgery of the National University of Health of Ukraine named after P.L. Shupika.
Main part. Training for surgical interns was organized in a mixed format, that is, theoretical disciplines in an online format; and the acquisition of practical skills, the study of which requires access to the patient in an offline format. Distance education was conducted in the remote conference system on the Zoom Video Communications platform, and exchange resources using the cloud services of the Google platform were used. The mailing was carried out via Viber messengers, cellular communication. The educational material fully corresponded to the approved curriculum according to the internship program for intern doctors and the specialty "Surgery". The content of the lecture material was presented as a PDF presentation. Seminar classes were conducted in the format of an online webinar, the results of test tasks and situational tasks were analyzed. Practical classes were limited to independent study of video materials of educational videos and operations. An interview, Viber messengers, chat, and e-mail were used for the final control of knowledge. In order to master the practical skills of operative interventions, interns in surgery were involved in the treatment process of the departments of the surgical clinical base of the department of the multidisciplinary city clinical hospital No. 6.
Conclusions. The optimal ways of learning in the intramural cycle of internships in surgery in a mixed form in martial law conditions are a combination of teaching the theoretical part in an online format and acquiring practical skills in mastering surgical interventions through the integration of interns in an off-line format into practical activities in surgical departments of the clinical base of the Department of General and Emergency Surgery of of the National University of Health of Ukraine named after P.L. Abstract: Анотація. Мета: висвітлити сучасні погляди на передопераційну підготовку при перитоніті. Об’єкт і методи дослідження. Протягом 2021 р. в хірургічному відділенні Броварської багатопрофільної клінічної лікарні проліковано 360 хворих з гострим перитонітом різного генезу (186 (51,7%) жінок та 174 (48,3%) чоловіки). Результати. У клініці розроблена методика лікування пацієнтів із перитонітом. Передопераційний період повинен тривати не довше 2–3 год та проходити паралельно з діагностичною програмою. При підготовці хворого до хірургічного втручання необхідно провести катетеризацію сечового міхура та контроль кількості сечі, введення назогастрального зонда, катетеризацію центральної вени, корекцію гомеостазу та стабілізацію стану хворого шляхом проведення інфузійної терапії, антибактеріальну терапію, адекватне знеболення, очисну клізму. Для визначення ризику летальності в передопераційний період оцінювали стан хворих за модифікованою нами шкалою APACHE II. У 5 летальних випадках показники модифікованої шкали APACHE II були в діапазоні 8–28 балів. Висновки. Внаслідок виражених явищ ендотоксикозу всі хворі з перитонітом підлягають обов’язковій передопераційній підготовці тривалістю 2–3 год, яка спрямована на поліпшення показників гомеостазу та від якої багато в чому залежить результат лікування.2022-12-01T00:00:00Z
